Ability B4 Disability (AB4D) have developed the attached Carers questionnaire (in word and easy read format). The questionnaire provides Carers with an opportunity to share their thoughts on the impact of Cost Of Living, Social isolation and access to GPs for carers (paid and unpaid). Please note the closing date for submission is 30 June 2023.
