Better Care Together describes a new way of working. Behind this is; For Thurrock in Thurrock: an alliance of commissioners and providers of health, social care, hospital trusts and community, voluntary services. 

What’s Changing?

Starting in Autumn 2018 in Tilbury and Chadwell and continuing in to the new year you will already see changes to the way you are supported.

These Changes include Community Led Support, more health professionals within your doctor’s surgery, Wellbeing workers, more use of technology to enable access to your records and make decisions about your care faster.

What else?

• There will be more opportunities to test your health. We want to help you to identify any health problems earlier and support people better with Long-term health conditions.
• Making communities stronger with more support, services and small (micro) businesses supporting you and your families, in the place where you live.
