Annual health checks are for adults and young people aged 14 or over with a learning disability. In order to support this, the CCG has pulled together a pre-questionnaire to help prepare people with learning disabilities before their annual health check.
This should help them to prepare what questions to ask, desensitise some anxieties and help them get a better outcome during the appointment.
